Guiding principles

  • Services, policies and practices allied to the provision of Older People’s Medicine will be reviewed and updated to ensure all is appropriate and relevant to Older People’s needs, and to ensure any form of age discrimation is eliminated, with NHS services only being provided on the basis of clinical need (NSF Standard 1)


  • The service will be patient centred, and treat older people as individuals, enabling them to make choices about their own care (NSF Standard 2)


  • EHT will work with partners across the health and social service community to prevent unnecessary hospital admission and to design effective rehabilitation services to enable early discharge from hospital and to prevent premature or unnecessary admission to long term residential care (NSF Standard 3)


  • Older people’s care at EHT will be delivered through appropriate specialist care and by hospital staff who have the right set of skills to meet their needs (NSF Standard 4). The Older People’s professionals will actively participate as part of the overall programme for developing specialist Older People’s skills amongst a wider staff base


  • As part of the Division of Medicine and A+E, the Older People’s Service at EHT will work closely with medical colleagues to ensure appropriate models of acute care are in place to manage emergency medical flows. Similarly, the potential for improved work between surgical and Older People’s Services will be addressed, with Fractured Neck of Femur Services offering a first priority


  • The Older People’s Department at EHT will take action to prevent strokes working in partnership with other agencies and sub-specialty services where appropriate (NSF Standard 5). At EHT, the Older People’s Service will work closely with Neurologists and nursing colleagues to ensure the highest standard of care is delivered


  • EHT will work in partnership with councils, and voluntary groups to take action to prevent falls and reduce resultant fractures or other injuries in their populations of older people (NSF Standard 6)


  • EHT will work with WLMHT to ensure older people who have mental health problems have access to integrated mental health services, provided by the NHS and councils to ensure effective diagnosis, treatment and support, for them and their carers (NSF Standard 7)


  • EHT will work with organisations across the health and social care community to promote health and active life in older age (NSF Standard 8)
